　　 1、 沒有　0 none　溫和　1 mild　中等　2 moderate　嚴(yán)重　3 severe
　　 2、 間歇的　intermittent　恒定不變的　constant　換藥時(shí)　at dressing change

　　八、感染　Infection

　　 1、 疑似　suspected   
　　 2、 傷口拭子送檢　wound swab sent 
　　 3、 確診病菌感染　confirmed
